Play for Patrick – Heart Screen Open for Registration
We’re hosting our next heart screening at Apple Valley Senior Center on January 10, 2026!
Screening times: 9:00AM – 3:20PM
Volunteer shifts: 8:00 AM-12:00PM | 12:00PM – 4:00PM

Participants:
Find everything you need to know here, including consent forms, what to bring, and what to expect the day of the screening. (If you are a member of a team participating in the tournament, please use the code provided by your team manager to register during your team’s timeslot.)
Volunteers:
