Participants MUST have a completed paper copy of their Heart Health Survey to enter the heart screening Student Check-In station at their designated sign-up time.  If you need to reprint your registration, please follow the instructions here:

• Go to our screening schedule and login with the same credentials used to register the participant.
• Under "My information", click "My Health History Forms"
• From the drop down, choose the participant
• click the "Print Form" button.
