​To register a 14-24 year old to be screened, follow the below instructions. Note - if you have an event code, DO NOT use the "Redeem Invitation" tab. Enter it at step 3.

1. Click on the "Participate" button below and select an event.
2. Click the "Participant Registration" button.
3. Register for the site and select "events". If you have an event code enter it on this page to view the event.
4. Select the appropriate gender to view available time slots OR if you have already filled out a registration form, click the "I have already filled out a registration form" and choose the participant.
5. Select a time slot and click "Sign up".
6. You will receive an email confirmation of your registration.
7. Print out the Heart Health Survey from the "My Heart Health History" tab and bring to the screen!
