Participants MUST have a completed paper copy of their Heart Health Survey to enter the heart screening Student Check-In station at their designated sign-up time.  If you need to reprint your registration, please follow the instructions here:

• Go to our screening schedule and choose "Manage your Registration"
• Request the magic link and click it when it arrives in your mailbox.
• Under the screening you are registered for, click "Print Health history"
